Workshop on the Evaluation of a mHealth Platform at IEDCR

A dissemination workshop was held at IEDCR today (28/01/2019) on the Evaluation of a mHealth Platform for Diarrhoel disease decision- support in Hospitals. Mr Zunaid Ahmed Palak, State Minister for ICT Division, Ministry of Posts, Telecommunications and Information Technology was present as a chief guest, Professor Dr. Nasima Sultana, Additional Director General, Directorate of Health Services was present as a special guest and Professor Meerjady Sabrina Flora, Director, IEDCR presided over the workshop.  Among others high officials from Directorate of Health Services, Scientists from IEDCR, icddr, b and University of Florida participated in the workshop. The research findings were presented by Professor Meerjady Sabrina Flora, Director, IEDCR; Dr. Ferdousi Qaderi, Senior scientist, icddr,b, Dr Aushraful Islam, Scientist, icddrb and Dr. Eric J Nelson of University of Florida.

IEDCR along with icddr.b and University of Florida conducted the research in 10 hospitals in the country. The project focused   specifically in developing mHealth solutions for diarrheal disease especially for two reasons. The first reason is that diarrheal disease globally remains the second leading cause of death for children less than 5 years of age. Barriers to combat this problem include poor adherence to guidelines for rehydrating children and outbreaks often outpace current epidemiological tools. The second reason is that diarrheal diseases, including cholera outbreaks, are a model system to develop and test mHealth solutions that can be adapted to more complex chronic and acute diseases in Bangladesh and globally.

The project was developed to compare a paper versus a smartphone decision-support tool in a cluster randomized controlled trial (cRCT) to determine the impact of the method of decision-support on the use of IV fluids and medications. The smartphone tool was an adaptation of the paper-based World Health Organization guidelines, and is called the ‘Rehydration Calculator.’ The cRCT was conducted at ten government hospitals in Bangladesh over 4.5 months. A 6-week pre-intervention period established a baseline, and in the intervention, hospitals were randomized to use the paper versus smart phone tool by the admitting physician Standard of care was practiced at all sites, with the exception of the decision-support. Data were collected on demographic information, clinical assessment, treatment and outcome of the disease. The findings are encouraging and will reduce the uses of antibiotics to improve antibiotic stewardship, and the assessment of dehydration ultimately decrease the unnecessary intravenous fluid use. IEDCR with its partners will try to replicate the mHealths platforms in other hospitals of the country.

Speaking on the occasion as a chief Guest Mr. Zunaid Ahmed Palak, MP, honorable state minister for ICT ministry emphasis on the use of digital technology and assure that support will be given to establish a world class Health Tech Laboratory.  He also mentioned that by using this platform we can serve the people of the country   to fulfil the dreams of honorable Prime Minister’s digital Bangladesh. He urges scientists to utilize the country wide digital platforms to better serve the nation especially for the life threatening diseases. He also added that he is very happy and proud to see expert scientists of Bangladesh working with international scientists using IT technology to solve medical diseases of Public health concern for proper management to save lives.
